Dear Customer, While it is our priority to make sure that your project goes smoothly, situations do arise. This site will represent your most useful tool, to process your claim quickly and easily. We ask that you do so as soon as possible; O & S will not perform any service(s), without a claim form. To process a claim, you will need to have the following information available: · Your Name · Detailed explanation of concern Click the HERE to download or copy a claim’s form. No claim will be processed without a claim form. Fax (678.489.3610) or Email the completed form to: warrantyclaims@osinstallations.com . You will be contacted by an O & S customer service representative (by email or phone), after your claim has been reviewed (usually within 24 hours Monday - Friday). If your claim is warranted, an appointment will be scheduled to inspect the concerns noted on your claim form. Most problems can and will be taken care of at the time of the inspection, if it is deemed warranted. If this is not the case, no decisions will be made until the O & S Flooring Inspection Report has been processed. It is at that time, you will be contacted to schedule the repair. If O & S Installations, LLC has determined that a replacement and/or repair is needed, then a scheduled install time will not be set until the material has arrived at your installation site (this “does not” include material, which was not on site at the time of installation - see Section "Material" of your Service Agreement). Remember only the affected area will be replaced; unaffected areas “will not” be replaced (See Our Warranty).
